LiveWell Housing offers temporary supported accommodation with person-centered support. We help individuals address contributing factors to homelessness, whilst working towards developing their independence.

Referral to move in Process:
It all starts with a referral
Referrals can be made by your support worker, universal credits work coach, GP, mental health worker or someone who is supporting you.
If you wish to apply for housing please complete our referral form, Risk Assessment and Self Medication form and send it to info@livewellsocialhousing.co.uk
Once we have received your referral form, we will aim to respond back with a decision within 24 hours or sooner if you need urgent housing and meet our criteria.

Getting to know us
Once we have assessed your application, we will give you call to introduce ourselves and what we do.
We will then invite you to meet one of our housing officers who will go through a full assessment at our office or at your support workers premises.
At all stages, you are able to ask question to make sure our services are right for you and that you meet our criteria.

Moving in
Once we have completed our assessments we will award accommodation at a suitable home, we will then organise a moving date with yourself and your support worker.
You will be assigned a LiveWell Housing support worker, who will meet you and move you into your new accommodation.
Your support worker will be your regular contact, who will create a care plan whilst you stay with us, and will also deliver regular key work sessions and key contact.
